Retainers are the least glamorous part of orthodontics and the part that determines whether the result lasts. Teeth are held in place by bone and by elastic fibers in the gum tissue, and after treatment those fibers pull for months, while bone remodels for longer. On top of that, teeth drift naturally throughout life — lower front teeth crowd in adults who never had braces at all. Retention is not a grace period after treatment. It is maintenance, indefinitely.
The most common adult case at our San Marcos office is a patient who had braces as a teenager, stopped wearing the retainer, and is back twenty years later for the same lower incisors. Retreatment works, but wearing a retainer at night is dramatically cheaper.
Clear removable retainers. A thin, transparent tray made from a scan of your finished teeth. Nearly invisible, comfortable, easy to wear at night. They cover the biting surfaces, so they are also the type most easily thrown out with a napkin at a restaurant.
Hawley retainers. Acrylic plate with a wire across the front teeth. Very durable, adjustable, and long-lived — a well-cared-for Hawley can last many years. More visible, and slightly bulkier to speak with at first.
Fixed (bonded) retainers. A thin wire bonded behind the front teeth. Nothing to remember, which makes it the most reliable option for high-risk cases such as severe original crowding, rotated teeth or a closed gap between the upper front teeth. Requires careful flossing with a threader, and periodic checks in case the bond loosens.
Many patients end up with a combination: a bonded wire behind the lower front teeth and a clear retainer worn at night on top.
Our protocol is full-time wear for the first weeks after treatment as directed, then nightly, then nightly for life. "For life" surprises people, so here is the reasoning: the studies on post-treatment relapse are consistent, teeth shift throughout adulthood, and one night per week is not enough to hold a result once fibers and bone have been changed. A clear retainer worn while you sleep costs you nothing in daily life and preserves a two-year investment.
Rinse it every time it comes out and brush it with a soft brush and cool water. Never use hot water, the dishwasher or a car dashboard — heat warps the plastic and a warped retainer no longer fits. Soak occasionally in a retainer cleaner, not in mouthwash, which can discolor it. Keep it in its case whenever it is not in your mouth; dogs find retainers extremely attractive and pets are a leading cause of replacement. Bring it to every appointment so we can check the fit.
If it feels tight after a break in wear, that is your teeth having moved. Do not force it — call us.
Time matters. A retainer replaced within a week or two usually still fits the teeth as they are. Wait a few months and the teeth will have shifted enough that a new retainer will not seat, and correcting the drift first turns a simple replacement into limited orthodontic treatment. We take a fresh scan and make a replacement, and we can do it whether or not your original treatment was with us — bring any records or old retainers you have.
How much does a replacement retainer cost? It depends on the type and whether new records are needed. Call us and we will quote it before you come in.
Can I get a retainer if I had braces somewhere else? Yes. We scan your teeth as they are today and make retainers from that.
Is a bonded retainer better than a clear one? Better for reliability, since there is nothing to forget, but it demands more careful cleaning. For many patients the ideal is one of each.
My retainer no longer fits. What now? Come in. Depending on how much the teeth moved, we either make a new retainer or discuss short limited treatment to recover the alignment first.
Do I need a retainer after clear aligners? Yes. Retention requirements are identical after aligners and braces.
